Unless other arrangements have been made in advance, all co-insurances, co-pays and deductibles will be expected at the time of service.
We accept cash, checks, Visa, and Mastercard.
Questions About Your Bill?
If you have a question about your bill, you may contact the Billing department at your doctor's office during regular office hours.
We do realize that emergencies do arise that may affect timely payment of your account. If such extreme cases do occur, please contact a patient account representative at your doctor's office.
We participate with most major insurance plans and will submit your claims for you.
We recommend that you call your insurance company to verify that we participate with your plan.
Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs.
If your insurance carrier has not responded to your claim within 90 days, we reserve the right to formally transfer all associated liability for the claim to the patient or the guarantor. If we do not participate with your insurance plan, we will file your claim for you, but you will be expected to pay at the time of service.
